Most surprise dental bills do not come from dishonesty. They come from questions nobody asked before treatment started. A few minutes of direct questions at your consultation can prevent most of them, especially for larger work like implants. Below are ten questions worth asking, followed by five northern Utah practices to consider, starting with Stubbs Dental, which covers cost, financing, and insurance as part of its consultations.
Which questions prevent surprise dental fees?
1. Can I get a written estimate before treatment?
Always ask, and if you are uninsured or paying out of pocket, it is your legal right. Under the federal No Surprises Act, in effect since January 2022, dental offices must give self-pay patients a written good faith estimate of expected charges. If you request one, it should arrive within three business days. If your final bill comes in at least $400 higher than the estimate for a provider, you can dispute it through a federal patient-provider dispute resolution process, which must be started within 120 days of the bill.
2. Does the estimate cover every phase of treatment?
Implant treatment happens in stages, from extractions and grafting to surgery and final teeth. Make sure the estimate covers the entire plan, not just the first appointment.
3. Is my insurance verified, and is the office in network?
An in-network office has agreed to set fees with your insurer. An out-of-network office may bill you for the difference between its fee and what your plan pays. Ask the office to verify benefits before treatment.
4. Will you submit a pre-treatment estimate?
A pre-treatment estimate, also called a predetermination, asks your insurer to state in writing what it expects to pay. It is not a guarantee, but it reduces guesswork.
5. What could change the price during treatment?
Sometimes the plan changes once surgery begins, for example if more bone grafting is needed than the scan suggested. Ask what situations could raise the cost and how you would be told before any extra work is done.
6. Are imaging, sedation, and follow-up visits included?
These are the items most often left out of a headline price. Confirm whether the 3D scan, any sedation, post-operative checkups, and adjustment visits are part of the quote.
7. What does the warranty cover?
Ask what happens if an implant fails or a tooth chip, how long coverage lasts, and what conditions apply, such as keeping maintenance appointments. Get it in writing.
8. When is payment due?
Some practices collect in full before treatment, others bill by phase. Knowing the schedule lets you line up insurance, financing, or health savings account funds.
9. Are there fees for financing, cancellations, or missed appointments?
Financing may carry interest or setup fees, and some offices charge for late cancellations. Ask now rather than discovering it in a statement.
10. What is the refund policy if plans change?
If you pay in advance and part of the treatment is not completed, find out how refunds are handled and how long they take.

How do you protect yourself from billing surprises?
The best protection is a written estimate that covers the whole plan, paired with clear answers about insurance, payment timing, and what could change. Keep copies of every estimate and bill. Bring these ten questions to your consultation, and do not schedule treatment until each one has an answer you understand.
